Abstract
The implementation of the proposed project will be very helpful to the students and teachers of KNCHS-SHS who are experiencing sickness and are in need of immediate health care. The availability of the school clinic contributes to low percentage of absenteeism among students, due to the fact that they can easily access medicine with an accurate prescription from the school nurse. Also, for the next school year, there will be more students who will enroll in this school and therefore the construction of the school clinic is a must. The objective of the project is to provide the KNCHS-SHS students and teachers a good medical service through making them comfortable when experiencing illness. The total budget to successfully accomplished the proposed project is Php. 305,000.00. Our organization is the most appropriate to run the project because based on our past records, we have the expertise to effectively establish the renovation of school gymnasium, construction of Nutripan canteen, and automated tubig machine within the school vicinity in partnership with different NGO’S and constituents.

I. Context
Based on our observation, the unavailability of the school clinic amplifies the number of absences among senior high school students who are experiencing illness because they were not able to receive medical health care from the school. So, we initiated that the construction of school clinic in KNCHS-SHS is important because it reduces the rate of absences and contributes to the progression of school.

III. Project Justification

A. Problem Statement
The proposed project is to resolve the problem of the KNCHS-SHS students and teachers about the unavailability of school clinic when they need health care such as medication about their illness or sickness.
B. Proposed Approach
The construction of school clinic is the type of intervention used to address the problem of the KNCHS-SHS students and teachers regarding medical health care.
C. Priority Needs
The implementation of the proposed project will be very helpful to the students and teachers who are experiencing sickness and are in need of immediate health care. The availability of the school clinic contributes to low percentage of absenteeism among students, due to the fact that they can easily access medicine with an accurate prescription from the school’s nurse. Also, for next school year, there will be more students who will enroll in this school and therefore the construction of the school clinic is a must.

D. Project Goal and Objectives
Goal:
The project aimed to be able to accommodate and suffice the students’ needs for medical health care when they are not feeling well instead of going home and having lots of absences in school.
Objectives:

  1. For the KNCHS-SHS students and teachers to attain immediate help when they need health care for medication
  2. To provide the students and teachers a good medical service through making them comfortable when experiencing or suffering illness.

IV. Project Implementation
The PTA Officer together with the administration shall assess and lead the plan for the construction of the school clinic. The school PTA treasurer shall handle the donated and obtained money for the project as provided in the governing policy of the school. This project shall be taken full responsibility by the PTA from October 8, 2019 until the end of the operation. The school administration needs to ask donations and assistance from the Department of Education, City Government/Administrator and City Health Office to successfully establish the construction of the school clinic.

V. Implementing Organization
Our organization is the most appropriate to run the project because based on our past records, we have the expertise to effectively establish the renovation of school gymnasium, construction of Nutripan canteen, and automated tubig machine within the school vicinity in partnership with different NGO’S and constituents.
VI. Risk Management
The risk in the implementation of the project is that there might be a delay in the construction because there is lack of financial support and also when there is mismanagement or failure amongst the school administration.
VII. Monitoring and Evaluation
The project management team will monitor the project’s progress through a constant visit in the project location January 7, 2020 to June 4, 2020.
